Description
Purpose of the role:
The clinical pharmacist will work remotely within their clinical competencies to provide expert support in medicines management across GP practices and Primary Care Networks (PCNs). The postholder will have substantial primary care experience and be confident working autonomously, demonstrating clinical excellence and supporting the delivery of safe, effective, and high-quality patient care. By delivering consistent, high-standard clinics, they will help maintain strong relationships and demonstrate value to both customers and patients, contributing to the reputation and growth of the service.

They will use their experience to support colleagues where appropriate, sharing best practice and providing clinical input or guidance where needed. The pharmacist will collaborate with GPs, nurses, and other Allied Health Professionals to improve outcomes from medicines and promote a patient-centred approach to care.

Principal Accountabilities
– Deliver high-quality medication reviews and structured medication reviews (SMRs).

– Conduct long-term condition clinics within scope of competence.

– Process acute and repeat prescription requests, ensuring safe and appropriate prescribing.

– Review and action hospital discharge and outpatient clinic letters.

– Provide medicines information and advice to clinicians and patients.

– Undertake and lead clinical audits, share learning, and implement improvement actions.

– Support achievement of QOF and IIF indicators, and Local enhanced services through proactive medicines management.

– Provide training, mentorship and clinical guidance to the pharmacy team.

– Contribute to the ongoing review and development of clinical protocols and pathways.

– Work collaboratively with GPs or other healthcare professionals to optimise patient outcomes and reduce medicines-related harm.

Essential Skills & Experience
– Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council (GPhC).

– Minimum 2 years' post-registration experience in primary care or GP practice settings (on-site not remote experience)

– Completion of the CPPE Primary Care Pharmacy Education Pathway (PCPEP), or equivalent.

– Strong clinical knowledge across long-term conditions (including Hypertension, Respiratory, Heart Failure and Diabetes).

– Experience using GP clinical systems (EMIS or SystmOne)

– Excellent written and verbal communication skills.

– Strong organisational, analytical and IT skills.

– Demonstrated ability to support and develop colleagues.

– Ability to recognise personal limitations and work within professional competencies.

– Works effectively, flexibly, and enthusiastically within a team or independently.

Desirable Skills
– Independent Prescriber qualification.

– Evidence of quality improvement or leadership experience.

– Experience contributing to multidisciplinary team meetings and service development.
